Use this form to submit your weekly journal about the week's topic and your experiences. 

Print as many copies of this form as needed each week.  Complete it daily in blue or black ink (not pencil). Have your site supervising initial each supervised activity at least weekly.  To correct errors draw 1 line through the incorrect information.  Write the correct information near the incorrect information.  Date the change and place your initials beside it.  Do not use white out or scribble over incorrect information; the original documentation must still be legible.  You will submit a copy of all completed/initialed activity logs after week 1 and after week 4.  Keep all original weekly activity logs to submit to your course instructor at the completion of your fieldwork experience. 

Complete this form to set your personal goals prior to the beginning of each week of fieldwork and submit for grading.  At the end of each week, update the form with your progress toward your goals and submit for grading.
